Survival after out-of-hospital cardiac arrest varies across regions in the United States. With an increasing number of 
CARES state participants, there is a unique opportunity to present aggregate metrics by state, allowing for a better 
understanding of OHCA incidence, survival outcomes, and bystander intervention rates nationwide. Table 4 presents 
aggregate metrics for state participants that had at least 50% population catchment in 2025. A total of 20 states and 
the District of Columbia voluntarily agreed to participate in reporting these metrics.
The included states have a wide range of both population catchment (from 597,894 to over 35 million) and incidence 
rates (from 55.0–127.5 per 100,000; a 2.3-fold difference). There is also marked variability in community interventions, 
with bystander CPR rates ranging from 26.0–77.0% (an approximately 3-fold difference) and public AED use rates 
ranging from 6.0–19.6% (an approximately 3.3-fold difference), as well as patient outcomes (overall survival: 6.6–
14.1% (an approximately 2.1-fold difference); Utstein survival: 20.1–42.9% (an approximately 2.1-fold difference)).
Public Reporting of State Aggregate Metrics
CARES sincerely appreciates the willingness of state participants to voluntarily share this valuable information. 
Reporting of state-level variation in bystander intervention rates and patient outcomes underscores the importance 
of data collection for OHCA, promotes the sharing of best practices, and facilitates system improvements aimed at 
saving lives nationwide.
